Introduction Comprehensive Medication Management (CMM) is a collaborative patient-centered approach to medication optimization. We recently implemented telephonic CMM as part of multidisciplinary team within team-based at-home care program. The objective this study was describe the type and frequency therapy problems (MTPs) identified by pharmacists delivering program, well acceptance rates recommendations made rectify them. Methods conducted retrospective chart review all patients receiving program between December 12, 2018 31, 2019. classified MTPs based on framework developed Pharmacy Quality Alliance. Descriptive statistics were used determine quantity MTPs, recommendations. Results 307 who received CMM. Of these patients, 58% female (178/307) 96% White (296/307). Average age 72 years old (SD 13). On average, 4 MTPs/patient with 35% rate (431/1228) for their those accepted, able resolve 185 (43%) own while providers resolved remaining 246 (57%) MTPs. Acceptance more likely if spoken (38.4% vs 29.5%, relative risk [RR] = 1.30, 95% CI 1.10-1.54, P .002).
